Here’s What You Need to Submit
Video Transcribed: What are the requirements for the supplemental packet required by the OBN? I’m Ope Adegbuyi, a medical marijuana business lawyer in Oklahoma. I had spoken about the supplemental packet that is now required for every applicant, and every medical marijuana business applicant to submit.
So, within this application, you need to submit an affidavit. The affidavit needs to be completed by each member, and each owner. So, if you have an LLC, if you have four members, everyone needs to complete that affidavit.
The second thing would be, they would need either your articles of incorporation, your operating agreement, and all management agreements. So if you have, that’s if you have an LLC, if you have a corporation, you need your bylaws and other management documents that you have.
And then you would also have to complete an inspection assessment form. You might include pictures and documentation as attachments for this assessment form. And then you would also need the owner of the property’s information. You have to fill it out in the form, who owns the property.
And then you have to fill out the employee identification form. So it asks you who are the current employees in the business, what their addresses are, what their names are, and their phone numbers. And then you also have to submit a site plan for the medical marijuana registration certificate.
So, basically, you would basically have to draw your plan, each room location, and all of that stuff. And then the last thing you need is the certificate of occupancy. So you would need a certificate of occupancy for all structures where controlled dangerous substances will be maintained and employees and agents would be.
So that’s basically the permit for the building that you’re going to be either growing or storing the product. And this supplemental packet has to be completed by grows and processors. Dispensaries do not have to complete this packet.
